New Patient Information & Documents

OTHER THINGS TO BRING:

• New patient paperwork: This will allow us to make the best use of your time in our office.
• Insurance cards: If you have dental insurance, please bring your information with you.
• Please bring a complete list of their medications, both prescription and over the counter.
• Minors should be accompanied by a parent or guardian.
• If you need to be prescribed antibiotic premedication, please be sure to inform our staff.
• If you have any dental anxiety, please let the doctor know.

On your first visit, you will be greeted by our warm and friendly staff and proper documentation will be taken. A common first visit will start with x-rays being taken of your mouth and receiving a full examination from Dr. Robert Harvey or Dr. Rameek Mcnair. We will then do an evaluation of the health of your teeth, gums, and other oral instructions. Among other things, we will be looking for signs of oral cancer, tooth decay, and gum disease.If any health concerns are discovered, we will then discuss treatment options. At Aspire Dental Group we offer comprehensive care and a wide range of services, eliminating the need to refer you to other dental offices for care.

Services we offer include cleanings, cosmetic dental procedures, dental implants, periodontal care, laser gum therapy, and TMJ treatments. We also provide premedication prescriptions to our premed patients.
